The Out of Hospital Care Program comprises two pathways:
The Care Awaiting Placement (CAP) pathway seeks to enable eligible older people to safely transition from hospital to an interim placement in a small-scale domestic living environment where they receive support to finalise their arrangements for a permanent residential aged care placement.
The Ready for Home (RFH) pathway seeks to enable eligible older people to safely transition from hospital to an interim placement in a small-scale domestic living environment where they receive support to return to their home and avoid being prematurely admitted to residential aged care.
Without these services, older people may experience inappropriate lengthy stays in a public hospital.
Eligible older people include those who have care needs relating to physical frailty, cognitive impairment and dementia with some people requiring more complex care arrangements related to behavioural and psychological symptoms of dementia (BPSD).
The service provider must be a registered provider, or in the process of becoming a registered provider, with the Aged Care Quality & Safety Commission and will be required to implement and maintain compliance with the Aged Care Quality Standards including Standard 5 Organisations Service Environment.
The service provider could also be a registered provider with the NDIS Quality & Safeguard Commission, in which case it will be required to implement and maintain compliance with the NDIS Practice Standards Core plus High Intensity Daily Personal Activities modules.
Services are to be provided in alignment with the Charter of Aged Care Rights and Standard 3 (Clinical Safety Standard) of the National Safety & Quality Primary & Community Care Standards.
Consideration will be given to service providers who are unable to provide accommodation but can provide a fully staffed, small-scale service in a NT Government owned facility.
The Territory is looking to engage with service providers as soon as possible on a panel contract for an initial term of 48 months, with 2 further options of 12 months each.
